Nursing Facilities Providers,

Over the past several months DHCF and ESA worked diligently to transform the Medicaid application and renewal process for new applicants and current beneficiaries. Less than a year ago, DHCF and ESA met with many of you to discuss Medicaid eligibility policies and to introduce Quickbase, a new system for application submission and tracking. During the training, many of you shared your current challenges and concerns with the process. Based on the information received, DHCF and ESA established a workgroup to address your concerns. As a result, we have made significant changes and process improvements to the application and renewal process to include leveraging electronic data sources to verify information, submitting applications and renewals through Quickbase with the ability to review application/renewal status in real time, and developed new forms and fact sheets to educate staff on process and policy changes. Lastly, I would like to thank those of you who were able to attend the July 26, 2017 training. We had over 80 attendees.

Attached are the formed discussed during the training which include the following: Note: All of these documents are posted on to Quickbase

  1. Medicaid Policies and Procedures Training for Nursing Facilities 7-26-17 Presentation
  2. Updated Request for Action Form (DHS 1346)
  3. Updated Statement of Patient Payability (DHS 1445)
  4. Affidavit of Identity
  5. Nursing Facility Annual Level of Care Attestation Instructions
  6. Nursing Facility Annual Level of Care Attestation Form (To be completed at Renewal)

We are in the process of making revision to the QMB only application. The QMB only application will be posted to Quickbase once complete. ”

Thanks,

Danielle Lewis Wright
